56 Year old Male
- Working as Principal of a High School
Social History :
- Non Smoker, Occasionally drinks alcohol
Family History :
- Father had T2DM and died of CVD at the age of 62 years
Physical Examination :
- BP: 148/94 mmHg
- Height: 172 cm
- Body weight: 80 Kg
- Have central obesity
Laboratory Investigations before 2nd Dec, 2023
- Hb1c: 8.2%
- FBG: 144 mg/dl
- PPG: 230 mg/dl
- Microalbuminuria: 216 mg/gm
Medical History :
- T2DM from last 9 years
- Hypertension from last 8 years
- 6 years back Undergone PTCA with DES
- Cataract Surgery done 2 years back
Medication History :
- Metformin- 1000 mg BD
- Dapagliflozin-10 mg OD
- Linagliptin-5 mg OD
- Glimepiride-2 mg BD
- Degludec-18 units s/c OD
- Atorvastatin- 20 mg
- Methylcobalamine-1500 mcg OD
- Telmisartan 40 mg + Cilnidipine 10 mg OD
- Clopidogrel 75 mg + Aspirin 75 mg OD
- Metoprolol 50 mg OD
Revised Treatment on 2nd Dec, 2023 with Addition of Imeglimin: 1000 mg B.D
follow Up after 4 month :
- Hb1c: 7.4 %
- FBG: 124 mg/dl
- PPG: 158 mg/dl
Post Follow up Changes in Medication :
- Imeglimin: 1000 mg BD
- Metformin- 500 mg BD
- Dapagliflozin-10 mg OD
- Linagliptin-5 mg OD
- Glimepiride-2 mg BD
- Degludec-14 units s/c OD
Experts’ View :
- Better Glycemic control with Hb1c decreased from 8.2% to 7.4% in 4 months of Imeglimin
- Decrease in Basal insulin dose
- Half the dose of Metformin
- No G.I. related adverse effects
- No Hypoglycemia
- Weight Neutral
follow up Advice :
- Imeglimin: 1000 mg BD
- continue for next 6 months
